Our goal is to provide patients with a medical home.
We build our services to meet the needs of our patients and surround them with the care and
resources they need to live healthy and full lives. All physicians serving
our adult population are Board Certified in either Family Practice or Internal Medicine.
AMSHO: Adult Medical Services at Hotel Oakland (AMSHO)
is a satellite clinic of AHS that is uniquely situated within the same building as
two adult day health centers and provides much of the chronic as well as urgent care needs
of those elderly who attend the centers, as well as elderly who may come from other parts of the area.
Therefore, AMSHO services are designed to meet the health care needs of elderly patients.
AMSHO, with its smaller, more intimate care setting and its caring and dedicated staff, strives to provide comprehensive care to ambulatory adult and elderly patients, and aims for timely identification
and monitoring of particular elders at risk and those with debilitating disorders.
Services include:
- Chronic Disease management and complex co-morbid diseases for conditions such as:
- High blood pressure
- Diabetes
- Elderly functional assessments
- Activities of Daily Living Screening - assess patient's independent living skills and capabilities through a series of questions related to daily living activities
- Individual and group nutrition counseling, health education and weight management services
- Medication monitoring
- Preventive services such as:
- Cancer screening
- Diabetes screening
- Hepatitis B screening
- TB screening
- Tobacco use screening
- Adult immunizations
- Psychosocial assessments and psychotherapy
- Referrals
- Elderly and disabled ancillary services
- Social services: such as MultiServices Seniors Program, Paratransit, long-term care
- Management of hospice and dementia patients
- Advanced directives
