BART:   12th Street Station and Lake Merritt Station are a only a few blocks away.

Parking

We recommend parking in the underground garage of the Pacific Renaissance Plaza.
For patients, AHS will validate 1 - hour of parking at Pacific Renaissance Plaza only.
(Entrances on Franklin St. and Webster St. between 9th St. and 11th St.)

      •  Make a Right Turn onto 8th Street
      •  Make a Right onto Franklin Street
      •  The entrance to the parking lot is on the right next to the Bank of America

Instructions

The clinic is located at 275 - 14th Street, between Harrison St. and Alice St. in Oakland.

Please do not use the main entrance to Hotel Oakland. The clinic's entrance is on 14th St.

Documents to Bring
When Registering as an AHS Patient

• Identification that shows the following for the head of household and all family members enrolling
in Asian Health Services:

¤ Name

¤ Address

¤ Birthdate

Proof of identification may include one of the following:

      ¤  Passport or birth certificate
      ¤  Alien registration card
      ¤  California driver’s license or California ID card

• Social Security card

• Verification of income for all family members (head of household, spouse, and
children under 21 living at home).

Bring ALL of the following that applies to your family:

¤ Paycheck stubs for the last
2 months

¤  If not employed, a statement of
    support (such as room, board and
    cash contribution)

¤ Unemployment check stubs

¤ IRS tax forms from previous year

¤ GA or CAPI award letter

¤ Current bank statements

¤ Social Security benefit
award letter

¤ Notice of Action from Medi-Cal
office (if applicable)

¤ School grant/scholarship
award letter

¤ Car registration

• Name, address and phone number of employer(s)

• Proof of your address which may include one of the following:

¤ Current utility bills (PG&E or telephone bill)
¤ Rent receipt with address and signature of landlord

• Health insurance information (if applicable):

                 ¤  Medi-Cal card
                 ¤  Medicare card
                 ¤  insurance ID card and coverage plan booklet

• Name and address of any past or present health care provider seen
(physician/clinic/hospital)

• Any past or current medial records

• Current medications (if any)

• Minors (under 18) must be accompanied by a parent or legal guardian.
